Two points make a line. And a trajectory.
One on the ridge, one on the zygoma. You still work freehand, but backed by digital planning: this online course teaches you to decide both points in the plan and carry them from the screen to the bone, with a virtually operated prototype and a thin resin guide you print in your own office.

You already have cases
You have placed zygomatic implants and you have taken a hands-on course. This course does not start from zero.
You want the trajectory first
The implant position decided in the plan, in every plane, instead of in the moment.
You want precision you can repeat
The same method in every case, from quad zygoma to rescue cases, with a double check at each step.
Haven't placed a zygomatic implant yet? This is not your first step.
Anatomy lies. So do your eyes.
The surface of the zygoma doesn't tell you what it is like inside. And with everything exposed, the starting point looks obvious, yet the right one is almost always more distal than it seems. Zygomatic implants can't be trial and error.
Depends on the eye of the day
It works until the case where the anatomy fools you. And that is the case where precision matters most.
Possible. Reliable? Not always.
On the zygoma, a small deviation at the entry grows along the length of a long implant.
Freehand, backed by digital planning
You keep the feel and control of freehand work. A thin guide marks the planned direction on the bone and comes off; the smart prototype gives the depth, and linear measurements are the double check.
“It isn't guided surgery. But it might as well be.”Fernando Giovanella
Module 1, open on YouTube.
The big problem: why anatomy fools the eye, where guided surgery falls short on the zygoma, and how the SkinGuide concept answers it.

From the plan to the bone, in four steps.

The CBCT becomes a plan
You define the ideal implant position in every plane: the point of maximum anchorage in the body of the zygoma, the implant head from the prosthetic reference, and the safe limits of the orbit and the infratemporal fossa.

Two points, one line
The plan becomes geometry: point A on the ridge and point B on the zygoma. Two points make a line, and the line is the trajectory.

The virtually operated prototype
The osteotomy is done on the computer, and the prototype comes out of the printer with the path already in it. It is a map to consult, not a model for trial and error.

The SkinGuide Zygoma
A thin resin guide, designed on the same plan and printed in your office. It seats on the bone, draws the direction and comes off. The smart prototype gives the depth. Then you check again.
The SkinGuide Zygoma: designed on the 3D model, printed in resin, seated on the prototype.
The full program. Five modules.

MODULE 1The big problemThe class that opens the course
- Overview of the SkinGuide concept
- The “clinical eye” technique? Really? (Spoiler: anatomy lies. So do your eyes.)
- Internal vs. external zygomatic anatomy: an inconvenient truth
- The “window” technique: a rarely useful solution to an almost non-existent problem
- Practical demonstrations of planning errors
- Guided surgery for zygomatics? Possible, yes. Reliable? Not always.
- From trial and error to a planned trajectory
- The SkinGuide concept: an alternative, simplified and powerful guide
MODULE 2Ideal positioning of the zygomatic implantWhere to anchor, and why
- The point of maximum anchorage in the body of the zygoma
- The implant’s relationship with the infratemporal fossa
- Indications for tetracortical anchorage
- Safe limits of the infraorbital rim; flat or deep floor anatomy
- Ideal implant head position: prosthetic reference, biomechanics, soft tissue, sinus risk and apical anchorage volume
- The risk of late soft tissue dehiscence, and how to reduce it
- Implant length by bone insertion and prosthetic connection type
- Implant position to avoid sinusitis
- All of it applied in virtual planning, step by step
MODULE 3The intelligence of prototypesThe simulator you didn’t know you needed
- Freehand training prototype vs. smart prototype
- Smart prototypes: the virtual osteotomy, advantages and benefits
- Designing the prototype before 3D printing
- Anatomical markings on the VPO (virtually operated prototype) for implant sizing
- Anatomical markings on the VPO for intraoperative landmarks
- Linear trajectories and notes for the day of the procedure
- Double check with linear references
MODULE 4SkinGuide: when the best path is already traced on the boneDesigning, printing and using the guide
- What the SkinGuide Zygoma is, how it works and how it is used
- Depth reference and double check with the VPO (virtually operated prototype)
- Biocompatible marker for drawing on the bone
- Designing the SkinGuide Zygoma virtually
- Essential anatomical areas and areas to avoid (undercuts, cavities, ridges)
- Software, concepts, common errors and advanced tips
- Printing in your office: precautions, resins and best practices
- Disinfection and sterilization
MODULE 5Clinical aspectsThe execution, step by step, with commentary
- The technical sequence, step by step
- Anesthesia, incision design and pencil marking
- Preparing the implant site with the SkinGuide and the smart prototype
- Zygoma kit and drill sequence
- Sinus membrane: perforate or preserve
- Preventing soft tissue dehiscence
